Preserve the customer's actual problem

Feedback loses value when every comment becomes a broad theme such as "communication." Keep a short, de-identified description of the event, channel, product area, and outcome. Link it to the approved case record so an owner can inspect the evidence without copying personal information into a spreadsheet.
Distinguish frequency from consequence
A common inconvenience and a rare severe failure should not compete on count alone. Record frequency, customer effect, reversibility, and confidence in the classification. Mark inferred themes as interpretations, not customer quotes.
Close the decision loop
Give each accepted action an owner, scope, and review date. Also record decisions not to act and the reason. Later sampling should test the exact failure the change was meant to address, rather than searching for any favorable metric.
Use customer service process improvement and customer update ownership maps to assign follow-up. The OMB standards for statistical surveys offer useful measurement principles. Customer Care Staff can maintain the register; the client decides product and policy changes.
