Start with the first broken expectation

Published September 8, 2026.

Do not assume every reopened case was closed too early. A customer may return because delivery changed, a refund had not posted, or a separate issue appeared. Read the sequence and identify the first point where the recorded expectation and the later outcome separated.

Use causes the team can act on

Keep categories plain: wrong answer, incomplete action, missing follow-up, unclear ownership, new event, or unrelated request. Allow reviewers to explain an unusual case rather than forcing it into a scorecard. One well-supported category is better than several guesses.

Leave with a testable change

Choose one recurring cause and adjust the smallest useful part of the workflow. The next sample should check whether that particular break became less common. Keep coaching private and route policy or system defects to their owners.

Connect the clinic to queue health reviews and customer update ownership. The ISO quality management overview offers general process context. Customer Care Staff can facilitate review, while the client retains policy and personnel authority.
